This nasty bug will delete your report or form in the Report/Form Designer when you click "Save" or "Save and Preview".
Cause: write permission of the report or form file is not set. The good part is a helpful error message saying you need write permission, but it doesn't say we just deleted your report or form.
Fix: Don't remove the record if the record update fails.

What happens here is there is no permission set and therefore no one can access the form. It's there, just not visible as you don't have permission to view it. You can retrieve the form in the db by forcing a permission value. I will see if I can add a warning if no permissions are set to prevent this problem.
